This full-time Speech Therapist (SLP) position is within the Outpatient Rehabilitation department at AMC Grafton. The role involves interpreting physician referrals, conducting comprehensive speech and language evaluations for patients presenting with various speech, language, voice, cognitive, and swallowing disorders. Key responsibilities include establishing and implementing individualized treatment plans based on evaluation results and functional goals, monitoring patient outcomes, and adjusting treatment plans as necessary. The therapist is also responsible for patient discharge planning, which includes providing home programs, patient/family education, community service referrals, and equipment ordering. Effective communication and collaboration with physicians and other healthcare teammates are crucial for optimal patient care. The role requires diligent documentation, adherence to billing policies, scheduling, and maintaining productivity standards. Additionally, the therapist is expected to engage in professional development, orient new staff, participate in co-worker evaluations, and may supervise non-licensed personnel and support student clinical education programs.
