About The Position

Elara Caring is seeking a compassionate and driven Speech-Language Pathologist (SLP) for a home health role in the Chicago Southland region. The role requires travel between patient homes in service areas including Tinley Park, Aurora, Joliet, Naperville, and surrounding communities. Requirements

• Master’s degree in Speech-Language Pathology
• Active Illinois SLP license (unrestricted)
• At least 1 year of clinical experience
• Valid driver’s license + reliable transportation
• Willingness to travel within coverage area

Responsibilities

• Evaluate and treat patients with speech, language, and swallowing disorders
• Develop and implement individualized plans of care
• Perform OASIS assessments and ongoing re-evaluations
• Collaborate with physicians and interdisciplinary teams
• Educate patients and caregivers for continued success at home
• Ensure accurate, timely documentation
