Shift Days, Salaried Scheduled Weekly Hours 40 POSITION PURPOSE: Evaluates and treats patients exhibiting or at risk for speech, language, fluency, voice, cognitive-linguistic functions and/or swallowing disorders.

Requirements

  • Master’s degree in Speech-Language Pathology required.
  • ASHA Certificate of Clinical Competence and State of Illinois Licensure in Speech-Language Pathology required within 6 months of hire.
  • Valid Illinois Vehicle Operator’s License required.

Responsibilities

  • Actively participates in patient evaluation conferences, with emphasis on discharge planning and referrals pertinent to continued rehabilitation and/or stabilization of clinical gains.
  • Evaluates speech, language, fluency, voice, cognitive-linguistic functions or swallowing.
  • Interprets test results, establishes and carries out Plan of Treatment (POT).
  • POT may include medical referral, speech, fluency, voice, cognitive-linguistic functions or swallowing therapy.
  • Performs therapy by setting goals, frequency and duration of therapy or upon physician order and communicates these to patient and patient’s family members.
  • Documents evaluation, recommendations and treatments through written reports and progress notes.
  • SLP dictates the report of findings and forwards to the original referral source or sends copies to party in which they are referring patient.
  • Promotes profession and department through participation in Health Fairs, trade shows, community presentations and community services organizations.
  • Prepares and submits appropriate paperwork to obtain funding for patients who qualify to Illinois Department of Public Aide (IDPA), DSCC, Department of Rehabilitative Services (DORS).
