Department: 37650 Navicent Health Medical Center - Inpatient Rehabilitation Status: Part time Benefits Eligible: No Hou rs Per Week: 0 Schedule Details/Additional Information: Days and weekends Pay Range $37.50 - $56.25 Major Responsibilities: Patient Assessments / Reassessments Performs appropriate patient assessments resulting in an effective treatment plan. Reassesses patients at appropriate intervals of time demonstrating the ability to observe treatment effect and modify the plan of care as indicated. Involves the patient and /or family in the assessment process and the formation of the care plan. Care Planning / Interdisciplinary Communication Provides care in accordance with physician’s orders. Demonstrates appropriate case management to ensure quality of care and best patient outcome. Communicates appropriately to physicians, nurses, case managers, and other members of the health care team to ensure care is collaborative and appropriate. Patient Outcomes Assesses and documents patient outcomes criteria at the initial and discharge evaluations. (all shoulder, back, incontinence, and anodyne). Speech Therapy Modalities / Patient Treatment Demonstrates knowledge of speech therapy modalities / treatments and performs correctly. Operates Equipment Safely and Correctly Demonstrates knowledge of and competency in using equipment pertinent to providing speech therapy services. Patient Education Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Includes the family in teaching as appropriate. Documentation Documentation meets current standards and policies. Documentation is legible. No inappropriate abbreviations are used. Documentation is completed in a timely manner. Medical Records are closed as per policy. Charge Capture Time is adequately documented in the assessments and daily treatment notes. Each charge bill adequately reflects all services provided. Demonstrates self-growth as a professional therapist. Engages in continuing education and professional growth opportunities.
