About The Position

Concierge Home Care is seeking a Speech-Language Pathologist (SLP) for a home health role in Sarasota, FL. Requirements

Active, unencumbered Florida Speech-Language Pathologist license
Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

Home health experience preferred but not required
Medicare OASIS documentation experience preferred

Responsibilities

Conduct comprehensive speech, language, and swallowing evaluations
Develop individualized treatment plans based on physician orders
Provide therapy focused on communication, cognition, and swallowing disorders
Educate patients and caregivers on techniques to improve functional outcomes
Collaborate with physicians and interdisciplinary care teams
Complete Medicare OASIS documentation as required
Document visits efficiently using AI-assisted tools
