Speech Therapist – Palm Bay, FL & Surrounding Areas In this role, you will serve as a vital partner in each patient’s plan of care, helping them achieve their highest level of function and safety within their home environment. You will focus on enhancing quality of life and communication abilities through evaluation, treatment, and rehabilitation. Working under a physician-directed plan of care—and in collaboration with patients and caregivers—you will support meaningful recovery and promote a better way of living.

Requirements

Current Speech Therapy license (State of Florida)
Commitment to the mission and values of Haven Home Health
Ability to work independently and exercise sound clinical judgment

Nice To Haves

Minimum of two (2) years of home health experience preferred

Responsibilities

Follow and uphold all agency policies and procedures
Evaluate patients and develop individualized treatment plans
Perform skilled speech therapy interventions as ordered by the physician
Improve or minimize communication and swallowing impairments
Collaborate with physicians and interdisciplinary team members on care planning
Participate in case conferences and coordinate patient care
Complete timely and accurate clinical documentation, including OASIS assessments
Monitor patient progress and adjust care plans as needed
Educate patients and caregivers to support continued progress at home
