Requirements
- Active MN Speech Language Pathologist License
- Master's degree in Speech Language Pathology
- CPR certification
- Reliable vehicle with valid license and insurance
Nice To Haves
- American Speech-Language-Hearing Association (ASHA) Certificate of Clinical Competence preferred
- Experience with seniors, geriatric dysphagia, and home settings, preferred.
Responsibilities
- Travel daily to assigned MN service areas to visit clients in their private homes
- Assess communication and swallowing disorders, then develop and implement personalized plans focusing on dysphagia, speech intelligibility, and cognitive-communication needs
- Provide interventions based on physician orders and educate caregivers in safe feeding techniques and communication strategies aligned with the care plan
- Maintain detailed records per MDH and payer guidelines, ensuring accurate billing through proper CPT code usage and modifiers
- Partner with the care team to review clinical progress, make adjustments to treatment plans, and ensure optimal patient outcomes
