Requirements
- Current Minnesota Speech Language Pathologist (SLP) license
- Valid driver’s license and reliable transportation
- Passion for patient-centered, one-on-one home care
Nice To Haves
- Experience with EMR preferred
Responsibilities
- Provide individualized speech, language, and swallowing therapy in clients’ homes
- Conduct diagnostic assessments and create tailored treatment plans
- Offer functional communication and cognitive-linguistic training
- Educate clients and caregivers on strategies that reinforce progress and safety
- Document services using EMR and communicate with the care team as needed
