About The Position

HealthPro Heritage is offering a PRN Speech Language Pathologist (SLP) opportunity in Trumbull, CT. This role involves providing Outpatient services to residents within an Assisted Living/Memory Care community, specifically an award-winning 93-bed assisted living and 27-bed memory care facility. The community focuses on keeping seniors connected to their passions and interests. Care is delivered either in an onsite gym or directly in the patient's room. Requirements

  • Must be a licensed Speech Language Pathologist - CCC in the state of Connecticut.
  • Degree in Speech Language Pathology from an accredited institution.
  • Valid state licensure as a Speech Language Pathologist, or license eligible
  • Current state licensure as a Speech-Language Pathologist.
  • Certification from the American Speech-Language-Hearing Association (ASHA) (CCC-SLP).
  • Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
  • Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

  • Conduct comprehensive evaluations of speech, language, voice, and swallowing abilities using standardized tests and clinical observations. Develop accurate diagnoses based on assessment results.
  • Implement evidence-based therapies for speech, language, voice, and swallowing disorders; monitor and adjust treatment plans; educate and support patients and families on disorders, treatment strategies, and daily communication improvement.
  • Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
  • Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.
