About The Position

We are seeking an experienced Speech Language Pathologist (SLP) to join a skilled nursing facility in Lake Worth. This role is ideal for an SLP who is comfortable working in a fast-paced SNF environment and focused on improving patient communication, cognition, and swallowing function. If you have prior SNF experience and enjoy working with a geriatric population, this is a strong opportunity with a supportive rehab team.

Requirements

• Active SLP license in the state of Florida (REQUIRED)
• Previous experience in a Skilled Nursing Facility (SNF) (REQUIRED)
• Strong knowledge of dysphagia management and cognitive therapy
• Experience with Medicare and SNF documentation standards

Nice To Haves

• Certificate of Clinical Competence ( CCC-SLP ) preferred

Responsibilities

• Evaluate and treat residents with speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans
• Document evaluations, progress notes, and discharge summaries accurately
• Collaborate with nursing, rehab, and interdisciplinary teams
• Ensure compliance with all regulatory and documentation standards
• Educate patients, families, and staff on therapy plans and progress
