About The Position

Franciscan Health Lafayette East Campus 1701 S Creasy Ln Lafayette, Indiana 47905 The Speech Language Pathologist I provides evaluation and treatment of the patient within the parameters of legal and professional guidelines, and hospital and departmental policies and procedures. This position evaluates and implements a plan of care for the provision of comprehensive speech and language services for individuals exhibiting dysfunction in the areas of speech and/or language. WHAT YOU CAN EXPECT Patient Care Performs comprehensive speech and language evaluations, treatment and related follow- up services including discharge planning. Patient Records Documents and submits patient reports and charges within the established timeframe. Goal Re-Assessment & Discharge Planning Completes appropriate goal re-assessment and discharge planning. Treatment Plans & Education Trains and educates patients and family members or others and documents understanding of treatment plan and goals. Patient Care Provides speech and language therapy according to standards of care defined by the state practice act and other professional and governing agencies. Coordination of Care Collaboration Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care. Treatment Plan Development Develops a treatment plan with measurable goals based on patient assessment. Speech & Language Therapy Provides speech therapy, including selection and use of assistive equipment.

Requirements

  • Master's Degree
  • Speech Pathologist (SPE) -
  • Cardiopulmonary Resuscitation (CPR) -
  • American Speech Language and Hearing Association (ASHA) -

Responsibilities
