Specialty Pharmacy Patient Care Coordinator
About The Position
Responsible for the overall coordination and processing of patient medication orders for the Medication Adherence Program. This shall include: new order deliveries, refill orders, new patient on-boarding, and medication adherence tracking. Patient Care Coordinators work with providers to address clinical documentation, while improving the pharmaceutical care and medication compliance of patients. Communicates with Medication Adherence Program and documents the calls in EMR system. Create SOPs and train new call center employees. Patient Care Coordinator may rotate through their assigned clinic based on operational need.
Requirements
- Five (5) years of medical or pharmacy related experience required.
- Will also consider candidates with an Associate's Degree in a science or health related field and three (3) years of medical or pharmacy related experience OR Bachelors Degree in a science or health related field and one (1) year of medical or pharmacy related experience.
- Excellent written/oral communication, interpersonal, organizational, conflict resolution, and computer skills required.
- Ability to work independently.

- National Pharmacy Technician Certification
- NYS Licensed LPN
- Medical Assistant Certification preferred.
