About The Position

This position will practice within the scope defined by the Ohio professional licensing board. By utilizing their pharmacy license, the Pharmacist will work with patients, providers, and other members of the health care team to provide patients with appropriate and beneficial comprehensive pharmacy services. In addition, this role supports development, implementation, and ongoing maintenance of specialty pharmacy clinical services in alignment with accreditation standards, including structured patient management programs, quality monitoring, and specialty medication distribution oversight.

Requirements

  • Degree from an accredited school of pharmacy and current Pharmacy License in the State of Ohio.
  • Successful completion of in-house training.
  • Current License in good standing with the Ohio State Board of Pharmacy.
  • Ability to act as a preceptor for pharmacy students.
  • CPR – BLS (basic life support) required.
  • Professional demeanor.
  • Ability to respect confidentiality of patient data.
  • Strong communication skills.
  • Ability to work as a team member.
  • Computer experience.
  • Ability to work with a culturally diverse group of people.
  • Must be willing to work flexible hours as required by position.

Nice To Haves

  • Knowledge of insurance and third-party payment systems preferred.

Responsibilities

  • Perform all duties of a pharmacist as legally specified in ORC 4729 and all other local, state, and federal regulations.
  • Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
  • Maintains knowledge of, and complies with, personnel, nursing, medical, and clerical policies and procedures as they pertain to pharmacist duties.
  • Maintains knowledge of loss prevention techniques and ensures security of the pharmacy and inventory at all times.
  • Maintains an awareness of developments in the community and pharmaceutical fields related to job responsibilities and integrates them into practice.
  • Participates in Quality Improvement activities.
  • Assists in training new employees.
  • Supervises pharmacy technicians and patient advocates while on duty in the dispensing department.
  • Demonstrates effective communication skills.
  • Responds to client/family/caregiver needs.
  • Utilizes on-going assessment skills.
  • Responds to crisis situations according to policy.
  • Participates in planning and problem solving.
  • Works as a member of the healthcare team by providing consultation and education to medical, dental and behavioral health and support staff regarding pharmaceutical utilization and other topics as needed.
  • Participates in organizational meetings as requested by supervisor including but not limited to staff meetings, provider meetings, and morning/mid-day huddles.
  • Establishes and maintains professional working relationship with other members of the health care team and facility personnel.
  • Participates in continuing education and professional growth.
  • Maintains workflow and order in the pharmacy while on duty.
  • Utilize dispensing software program to obtain and document all pertinent and legally required information to compound and or prepare, dispense, and perform drug utilization review for all prescription orders issued by authorized medical providers in accordance with all local, state, federal, and company regulations.
  • Perform Inventory management including but not limited to placing orders, auditing supplies, updating pricing, adjusting on hand levels of stock, verifying invoices.
  • Utilizes supplies in a cost-effective manner.
  • Effectively a document in patient’s electronic medical record as it pertains to job duties, both clinical and dispensing.
  • Interviews patients regarding past medication history and allergies, medical conditions and provides patients with instructions and counseling regarding medication management and chronic disease management.
  • Serves as a preceptor for pharmacy students and pharmacy residents as requested by supervisor.
  • Performs Clinical Pharmacy Duties (related to individual pharmacist’s competency level) such as but not limited to medication therapy management and disease state management patient / group visits.
  • Meets competency criteria at evaluations.
  • Ability to temporarily staff any service location of the HPWO Pharmacy Department in the event of staff shortages, while being compensated for travel between the primary work site and the temporary work site.
  • Supports compliance with specialty pharmacy accreditation standards.
  • Assists in preparation for accreditation audits and validation reviews.
  • Participating in after-hours clinical support as needed.
  • Participates in monitoring specialty adherence metrics and patient satisfaction trends.
  • Supports development and adherence to specialty medication packing and shipping procedures.
  • Participates in distribution error tracking and corrective action processes.
  • Communicates therapy recommendations and safety concerns to prescribers and care teams.
  • Contributes to reporting of performance measures required for accreditation.
  • Immediately notifies Director of Pharmacy Services of any instances that may affect the status of this individual’s license to practice pharmacy in the state of Ohio or meet the standards of conduct of HPWO.
  • Completes all other duties as necessary within the scope of this position as requested by the Director of Pharmacy Services.
