Locations Stanford Health Care What you will do Proficiency of PFC II skills PLUS Specialized tech knowledge (i.e. procedural/surgical area) OR Complex Care Coordination Performs 12 Lead EKG Schedules, processes, prepares, and dispenses cardiac device (zio/holter) monitor and tracks patient remotely for results Perform compression stockings measurements Prep and assist with in clinic Cardiovascular Medicine, Cardiothoracic Surgery, and Vascular Surgery procedures Monitor and report patient’s lab results, blood pressure logs and weight logs for cardiac medication management Schedules peer to peer insurance authorizations for cardiovascular clinic visits and cardiac and vascular diagnostics procedures/testing Coordination of care for complex cardiac echocardiogram and vascular lab scheduling/surgery scheduling Assist patient in coordination of procedure (i.e TAVR, catherization, interventional cardiology procedures, ablations, cardioversions, pacemaker, vascular surgery, cardiothoracic surgery, transplant, etc.) Understands proper use and applications for the following imaging modalities: radiology (CTA/PET, MRI), Echo, catheterization, carotid ultrasound, vein and arterial mapping Coordinates/triage cardiovascular hospital discharge appointments before discharge from inpatient Monitor and report patient’s lab results, blood pressure logs and weight logs for cardiac medication management Monitor, initiate and submits electronic prior authorization for cardiac medications These principles apply to ALL employees: SHC Commitment to Providing an Exceptional Patient & Family Experience Stanford Health Care sets a high standard for delivering value and an exceptional experience for our patients and families.
