About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona, including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services. CBI serves individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. As a premiere non-profit provider, CBI focuses on prevention, education, and treatment using evidence-based models. The Specialty Case Manager provides case management services in the community to patients with serious mental illness and/or those considered high risk due to acuity. This role involves planning, connecting, advocacy, coordination, and monitoring to help patients access health services, financial assistance, housing, employment, education, social services, and natural supports. The Specialty Case Manager assesses needs, develops and monitors individualized treatment plans, and ensures timely and accurate documentation. The position requires embracing the recovery philosophy, promoting a safe recovery environment, and implementing advanced techniques and interventions for high-risk individuals in their associated program.

Requirements

  • Highschool diploma or GED required
  • Current AZ Driver's License
  • 39-month MVR
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Arizona Fingerprint Clearance card
  • Jail Clearance (if applicable)

Nice To Haves

  • Associates Degree or higher in a field related to behavioral health
  • Recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year
  • 1-3 years of experience working with special populations (individuals experiencing serious mental illness, survivors of domestic violence, individuals determined high cost/high need)
  • 1-3 years of experience in a behavioral health position providing outreach and engagement activities
  • Peer Certification (if applicable)

Responsibilities

  • Provides case management services in the community to patients experiencing serious mental illness and/or those who are considered high risk due to their acuity level
  • Assists in planning, connecting, advocacy, coordination and monitoring to assist patients in gaining access to health services, financial assistance, housing, employment, education, social services and natural supports
  • Assesses needs
  • Develops and monitors individualized treatment plans
  • Documents services provided in an accurate and timely manner
  • Embraces the recovery philosophy and promotes a safe recovery environment for clients
  • Implements advanced techniques and interventions to serve the high-risk individuals in their associated program
