About The Position

The Bridge to Stability At Telecare’s Valley Vista PSH , we don’t just provide a roof; we provide a foundation for a new life. As a Specialty Case Manager , you are the subject matter expert on our team. While you provide general support to help residents maintain their housing, you also bring a "superpower"—a specialized knowledge base in Benefits, Education, or Employment —to help our clients navigate the complex systems that stand between them and total independence. In partnership with San Diego Behavioral Health Services and the San Diego Housing Commission, you will be a visible, supportive presence in the community, meeting residents where they are—whether that’s in their apartment, at a local coffee shop, or in our communal spaces. What You Will Do to Change Lives Be the Specialist: Serve as the team’s "go-to" expert in a specific area (e.g., navigating SSI/SSDI, vocational training, or educational enrollment). Champion Housing Retention: Provide intensive outreach and care coordination to help high-risk residents stay housed and stable. Foster Community: Organize and lead site-wide integration opportunities, from educational groups to community BBQs. Navigate the System: Use your personal vehicle to escort clients to essential appointments, ensuring they never have to face a daunting bureaucracy alone. Collaborate for Recovery: Work alongside a compassionate team to create individualized recovery plans that honor each resident's "health, hopes, and dreams."

Requirements

  • High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR Associate’s degree and two (2) years of experience; OR Bachelor’s degree in Social Services
  • Must possess at least 1 year of experience in the area of specialty served (e.g. Benefits, Educational, Employment, etc.)
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use your personal vehicle to attend meetings, etc.
