About The Position

Position Summary: Provides assessment and intervention to assist residents/families, and staff to assure that the emotional and psychosocial needs of residents are met/maintained on an individual basis and that resident’s rights are upheld. The position is responsible for a social work caseload in addition to administrative duties assigned.

Requirements

• Bachelor’s degree in social work from accredited educational institution
• State of Michigan licensure as Licensed Bachelor’s Social Worker (LBSW)

Responsibilities

• Seeks case supervision and consultation as appropriate.
• Participates in discharge planning, development and implementation of care plans, MDS and resident assessments.
• Complete MDS and resident assessments (RAP’s) as required by Medicare, federal, and state regulation.
• Obtains assistance for resident by referring him/her to community resources.
• Develop preliminary and comprehensive assessments of the social service needs of each resident that identifies the problems/needs of the resident and goals to be accomplished for each problem/need identified.
• Participate in discharge planning, development, and implementation of social care plans, MDS, and resident assessments.
• Maintain a productive working relationship with other departments and supervisors to assure that an interdisciplinary focus of resident care is maintained.
• Complies with federal, state, and local regulations, Marwood’s established policies and procedures, NASW Standards, and ethical guidelines.
• Performs other related duties as required and directed.
