Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee. Scope of Work: Social Worker on Flourish Health Clinic Flourish Health Clinical Service Delivery The Social Worker for the Flourish Health Clinic provides trauma-informed, field-based behavioral and medical health support to individuals impacted by substance use, mental health, housing insecurity, and barriers to care. As part of the interdisciplinary team, the social worker conducts mental health and substance use screenings, provides harm reduction education and supplies, offers brief interventions and crisis support (including de-escalation support), and coordinates referrals and follow-up planning to behavioral health, medical health and social services (such as mental health, recovery, medical care, housing, counseling, etc.). This role also supports harm reduction education, ensures accurate documentation of assessments and referrals in compliance with legal requirements, and contributes to student training and continuity of care through collaboration with community partners. We established our non-profit agency by merging three community organizations: Urban Case Management of Meharry Community Health Center, Continuous Treatment Team of Council of Community Services, and Mobile Crisis Team of the Crisis Intervention Center. This fusion resulted in an innovative, comprehensive, and multidisciplinary approach that addresses the specialized needs of individuals living with severe behavioral health conditions and challenges related to poverty. Responsibilities
