About The Position
Elite Home Health, a part of LHC Group and Optum, is seeking a Social Worker - Per Diem. This role involves providing medical social services under the direction of a physician and Interdisciplinary Group. The Social Worker will assist in understanding significant social and emotional factors related to the patient's health status and in the development of coping mechanisms. Requirements
- Master's Degree from a school of Social Work accredited by the Council of Social Work
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
- Licensed as a Social Worker from the State Board of Social Work (for AL, AR, CA, CO, DE, GA, ID, IL, IN, KY, LA, MA, MD, MI, MN, MS, NJ, NM, NV, OH, OR, SC, TX, WI,WV)
Nice To Haves
- Current CPR certification or ability to complete within 90 days of hire
- Licensed Social Worker in the state of residence
- 1+ years of social work experience in a health care setting
