About The Position

The Social Worker in the Mental Health Clinic practices at the independent or independent practice level. The Social Worker has specialized knowledge of practice typically related to assessment, diagnosis, and treatment of an outpatient Mental Health population.

Responsibilities

  • Maintains a large caseload within the program, facilitates individual, group, and family therapy.
  • Assesses the psychosocial and environmental needs that are secondary to or exacerbating the psychiatric problems.
  • Completes suicidal risk screens and assessments, psychosocial assessments, integrated summaries, clinical reminders, advanced directives, patient educations note, and other documentation, as indicated.
  • Possess the knowledge and ability to independently implement treatment modalities, provide psychotherapy for individuals, and groups.
  • Independently conducts psychosocial assessments.
  • Develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team.
  • Network both internally and externally to the facility and coordinates community-based services as may be necessary.
  • Knowledge and familiarity with Veteran's benefits and services relating to special programs, service-connected compensation, non-service-connected pension.
  • Provides case management as needed, which may include assistance with housing, transportation, food, or other resources.
