We’re seeking a compassionate and skilled Licensed Master’s Social Worker (LCSW) to support patients facing social, emotional, economic, and environmental challenges. In this role, you’ll conduct assessments, develop intervention plans, coordinate services, and collaborate closely with our healthcare team to ensure patients receive the support they need. You’ll also assist with discharge planning, connect patients to community resources, and complete Medicaid and Safety Net screenings as needed. This position also ensures high quality, patient centered social services through treatment planning, therapy modalities, and ongoing evaluation. We’re looking for someone who thrives in a fast paced environment, exercises strong critical thinking skills, and can work independently while supporting patients and families through challenging situations. Requirements

  • Master’s degree in Social Work
  • LCSW (State of Arkansas)
  • BLS HCP; Crisis Intervention Training within 90 days
  • 2+ years of related experience
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Responsibilities

  • Conduct interviews and comprehensive assessments with patients, families, caregivers, and legal representatives to identify social, emotional, and environmental needs.
  • Determine, prioritize, and coordinate internal and external services or interventions; provide referrals for transportation, housing, healthcare, and social or spiritual support.
  • Participate in case reviews and collaborate with the healthcare team to monitor, promote, and evaluate compliance with individualized treatment plans.
  • Support discharge planning by arranging follow up care, ensuring continuity with outside agencies, and completing Medicaid screenings and Safety Net applications when indicated.
  • Fulfill responsibilities for inpatient behavioral health, including monitoring service quality, completing bio psycho social assessments, developing measurable goals, providing therapeutic modalities, and participating in utilization management.
  • Complete required documentation within established time frames and assist with orientation, training, meetings, and other duties as assigned.
