Cadre Hospice seeks to deliver an exceptional end-of-life experience, one patient at a time, and raise the standard of hospice care. This PRN position offers flexible hours. The role involves joining an interdisciplinary team as a Licensed Masters Social Worker (LMSW) to provide comprehensive psychosocial support to patients and families facing end-of-life issues. The ideal candidate should have a strong background in social work, counseling, and palliative care, with a deep understanding of grief, loss, and bereavement. The candidate must be open to travel as necessary within the designated area.

Requirements

  • Master’s degree in Social Work (MSW) from an accredited program and a licensure for the state of Indiana
  • Minimum of one-year of experience in social work, grief counseling, bereavement support or related in a healthcare field, preferably in hospice or palliative care
  • Clean driving record, and valid driver’s license and auto insurance

  • Resource Referral and Coordination
  • Bereavement Support and Follow-up
  • Assist in providing comprehensive bereavement support to families and caregivers following the death of a hospice patient as needed
  • Any other duties as assigned by leadership
