The role involves joining an interdisciplinary team as a Licensed Masters Social Worker (LMSW) to provide comprehensive psychosocial support to patients and families facing end-of-life issues. The ideal candidate should have a strong background in social work, counseling, and palliative care, with a deep understanding of grief, loss, and bereavement. The candidate must be open to travel as necessary within the designated area.

Requirements

Master’s degree in Social Work (MSW) from an accredited program and a licensure for the state of Indiana
Minimum of one-year of experience in social work, grief counseling, bereavement support or related in a healthcare field, preferably in hospice or palliative care
Clean driving record, and valid driver’s license and auto insurance

Resource Referral and Coordination
Bereavement Support and Follow-up
Assist in providing comprehensive bereavement support to families and caregivers following the death of a hospice patient as needed
Any other duties as assigned by leadership
