Requirements

Master's Degree in Social Work.
Current licensure as a LMSW or LCSW-C.
Completion of JHACH's intern program or a comparable internship in a healthcare environment.
Exceptional listening, verbal, and written communication skills.
Proficiency in electronic medical record documentation.

Responsibilities

Conduct comprehensive psychosocial assessments to address the social and emotional needs that influence patient treatment and recovery.
Perform risk assessments, including evaluations for suicide risk, to provide informed safety recommendations to the medical team.
Facilitate referrals and connect families with valuable community resources, enhancing care coordination and reducing psychosocial stress.
Deliver therapeutic interventions such as crisis intervention, grief counseling, and psychoeducation, equipping patients with healthy coping strategies.
Collaborate with the healthcare team to develop tailored care plans that tackle psychosocial barriers, preventing discharge delays.
Maintain accurate electronic documentation and data collection, ensuring adherence to regulatory requirements.
Communicate effectively with team members regarding patient needs, interventions, and insights during meetings and rounds.
