Requirements

• Active Maryland Master Social Worker License (LMSW).
• 1+ year of healthcare experience (hospice preferred).
• Strong communication and organizational skills.
• Valid driver’s license and insured vehicle.

Responsibilities

• Assess psychosocial needs of patients and families.
• Provide counseling, crisis intervention, and grief support.
• Assist with care planning, advance directives, and community referrals.
• Collaborate with interdisciplinary team to ensure holistic care.
• Support end-of-life planning and family decision-making.
