About The Position
Provides casework and counseling services to Residents and their families.
Requirements
- Master's Degree in Social Work preferred.
- 1-3 years clinical experience with the elderly population preferably in a LTC setting
- MSW preferred
- Ability to handle multiple demands and deadlines
- Ability to communicate effectively
- Ability to utilize various software packages to carry out position responsibilities.
Responsibilities
- Experienced with skilled nursing (LTC), assessing the needs of patients/residents and families, providing support and guidance, developing and coordinating discharge plans, referrals to appropriate agencies, MDS, Care Planning.
- Facilitate discussion with resident/patient, responsible parties and staff regarding concerns. Facilitate meetings with responsible parties and support groups as necessary. Make notations in medical record. Maintain complaint log.
- Participate in designing, and on occasion present, in-services to Mozaic Senior Life staff regarding issues related to resident rights and psychosocial needs of the elderly. Supervise graduate and undergraduate students in social work and/or volunteers as time permits.
- Coordinate and complete investigation of any abuse allegations submitting written reports as requested.
- Represent the resident/patient in court matters when necessary.
- Other related duties as assigned.
