Social Worker - Palliative Care (part-time/day)
About The Position
As a member of the Palliative Care team, the Social Worker works to assure continuity and coordination of care for outpatient adult oncology patients/families. Services include but are not limited to; assessing patient/family strengths in regards to social, environmental, psychological, financial, ethnic, spiritual, and cultural influences related to living with cancer, providing one-to-one acute emotional support to patients and families, enhancing client access to resources, facilitating group and family conferences, participating in patient care planning rounds and supporting continuity of care.
Requirements
- Masters in Social Work degree from an accredited school required.
- Licensed Social Work Associate-Advanced (LSWAA) or Licensed Social Work Associate Independent Clinical (LSWAIC) required.
- Two years experience as a social worker in a healthcare setting preferred.
- Experience working with patients who have a chronic or acute illness, preferably in an oncology setting.
Nice To Haves
- Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW) with the Washington State Department of Health under Chapter 18.225 RCW preferred.
