About The Position

The Palliative Care Social Worker provides counseling and medical social work case management to the patient, their family, and caregivers. This position assesses the social and emotional factors related to the patients illness and evaluates the psycho-social response to treatment and level of adjustment. This position interprets to the family, patient, physician, and staff, the social and emotional factors related to illness, financial and community resources.

Requirements

  • Master degree in Social Work from an accredited college/university.
  • At least one (1) year of experience as a social worker in a health care setting.
  • Ability to work independently with a minimum of supervision.
  • Ability to listen and respond non-judgmentally with respect to others beliefs.
  • Ability to communicate effectively verbally and in writing.
  • Valid drivers license, proof of acceptable automobile insurance coverage, and reliable transportation as specified by Hospice of Humboldts policies.
  • Reliable mobile phone with plan that provides good local coverage.
  • Good computer skills. Proficiency in Microsoft Office applications and electronic medical record systems.

Nice To Haves

  • Current professional license to work as a LCSW in the state of California.
  • Proficiency in Spanish language, particularly spoken language

Responsibilities

  • Provides outpatient psycho-social evaluation for advanced illness patient population.
  • Assesses and responds to psycho-social, financial and legal concerns of patients and their families/support systems.
  • Assists patient and family in accessing and using community resources as well as successfully navigating social and healthcare systems.
  • Actively participates in regular interdisciplinary team meetings and support team members in overall understanding and care of patient.
  • Facilitates patient-family meetings for goal setting and advance care planning.
  • Provides emotional support for patients and families facing advanced illness.
  • Engages with patients and families using videoconferencing technology, as needed.
  • Documents in the electronic medical record in a timely manner and in accordance with established policies and procedures.
  • Participates in palliative care quality improvement initiatives.
  • Provides liaison function between the palliative care team and other community services as needed.
  • Conducts outreach and collaborates with referring providers and agencies and assists in enrolling patient in services.
  • Assumes other duties in support of Hospice of Humboldts needs.
