About The Position

Along with other core team members, the Social Worker evaluates develops an Individual Family Service Plan for each child and provides direct services to children with delays in the areas of physical, cognitive, communication, social, emotional, and adaptive development. The Social Worker acts along with other team members to instruct parents to meet with other team members to plan and evaluate the program, to problem solve, discuss progress of each child and prepare a transition plan when appropriate.

Requirements

  • Master's Degree in Social Work
  • NJ Social Work License
  • Valid Driver's License
  • Minimum of three years of experience in child focused setting and field experience in health care of special education setting
  • Sound background in normal child development
  • Knowledge of the educational and therapeutic value of an early intervention for handicapped children
  • Excellent communication skills, both written and verbal
  • Excellent interpersonal skills
  • Good planning and organizational skills

Responsibilities

  • Completes development evaluations that accurately and efficiently identify strengths and needs
  • Communicates effectively with families and other team members in oral and written formats, in formal and informal situations
  • Supports and enhances the familys ability to interact with their child in daily routines
  • Displays competency in writing providing strategies that are appropriate, including measurable outcomes, as part of the IFSP
  • Incorporates recommendations of other team and family members
  • Evaluates progress on an ongoing basis
  • Submits all required billing and verification logs, progress notes and productivity reports
  • All paperwork is legible, accurate and adheres to codes of professional ethics and is submitted in a timely manner
  • Selects and adapts appropriate activities, incorporating a variety of intervention model
  • Evaluates effectiveness of methods while meeting family outcomes
  • Takes responsibility for seeking supervision or consultation in relation to clinical performance
  • Actively contributes to improvement plans by effectively utilizing feedback
  • Assists in the training of others in the EI philosophy, as well as a variety of intervention models
  • Communicates job specific concerns and participates in developing solutions
  • Attends and participates in weekly in service staff meeting
  • Completes written work in a timely manner relevant to evaluation, IFSP development, intervention and transition summaries when appropriate
  • Majority of duties require walking, sitting, standing, bending, stretching, reaching, visual acuity and ability to safely lift up to 15 pounds
  • Natural environment settings including but not limited to the home and day care settings
