About The Position

The Social Worker (MSW) provides comprehensive psychosocial assessment, intervention, and support to patients and families to promote well-being, continuity of care, and optimal patient outcomes. This role focuses on addressing social determinants of health (SDOH), facilitating safe and effective transitions of care, and supporting patients through illness, adjustment, and life changes. The Social Worker collaborates with interdisciplinary teams and community partners to ensure patient needs are met through coordinated, patient-centered, and equitable care delivery.

Responsibilities

  • Conducts psychosocial assessments to identify emotional, social, and environmental needs that impact health outcomes.
  • Screens for and addresses Social Determinants of Health (SDOH) such as housing instability, food insecurity, transportation barriers, financial hardship, or caregiver stress.
  • Provides counseling, crisis intervention, and emotional support to assist patients and families with coping, adjustment to illness, and end-of-life planning.
  • Participates in interdisciplinary rounds to provide input on psychosocial context, discharge readiness, and post-acute support needs.
  • Collaborates with case managers, nurses, physicians, and external agencies to coordinate care and facilitate safe discharges.
  • Serves as a patient and family advocate in discussions related to care goals, ethical concerns, and advanced care planning.
  • Connects patients and families with appropriate community-based services, including long-term care, rehab, hospice, and home support programs.
  • Assists patients in accessing public benefits and resources such as Medicaid, SNAP, SSI/SSD, and transitional housing programs.
  • Develops individualized discharge plans that integrate psychosocial needs and available community supports.
  • Maintains updated knowledge of community resources and builds relationships with social service agencies.
  • Documents all assessments, interventions, and discharge planning activities in accordance with regulatory and organizational standards.
  • Participates in quality improvement initiatives to enhance psychosocial care and coordination effectiveness.
  • Upholds patient confidentiality and professional standards consistent with social work ethics and hospital policy.
