About The Position

The Social Worker (MSW) provides comprehensive psychosocial assessment, intervention, and support to patients and families to promote well-being, continuity of care, and optimal patient outcomes. This role focuses on addressing social drivers of health (SDOH), facilitating safe and effective transitions of care, and supporting patients through illness, adjustment, and life changes. The Social Worker collaborates with interdisciplinary teams and community partners to ensure patient needs are met through coordinated, patient-centered, and equitable care delivery. This role is specific to the outpatient Prenatal clinic. Essential Functions: Conducts psychosocial assessments to identify emotional, social, financial and environmental needs that impact pregnancy outcomes. Screens for and addresses Social Drivers of Health (SDOH ) such as housing instability, food insecurity, transportation barriers, financial hardship, or caregiver stress. Provides psychosocial support and intervention to assist patients with adjustment to pregnancy and post-partum needs. Collaborates with OB multidisciplinary team to coordinate care. Participate in interdisciplinary rounds to provide input on psychosocial context. Connects patient with appropriate community-based services including housing assistance, food programs, legal services, domestic violence shelters, substance-use treatment and mental health providers. Maintains updated knowledge of community resources and builds relationships with social service agencies. Documents all assessments and interventions in accordance with regulatory and organizational standards. Participates in quality improvement initiatives to enhance psychosocial care and coordination effectiveness. Upholds patient confidentiality and professional standards consistent with social work ethics and hospital policy.

Requirements

  • Master’s degree in social work (MSW) from an accredited program required.
  • Active New Jersey LSW or LCSW required.

Nice To Haves

  • Prior hospital-based or medical social work experience strongly preferred.
  • Spanish speaking preferred
  • Excellent communication, negotiation, and conflict-resolution skills.
  • Strong understanding of social drivers of health (SDOH) and their impact on patient outcomes.
  • Proficiency in electronic health records and data systems (Epic preferred).
  • Ability to collaborate effectively in a fast-paced, interdisciplinary healthcare environment.
  • Commitment to continuous improvement, equity, and patient-centered care.
