We are seeking a dedicated Social Worker to provide comprehensive psychosocial assessments, crisis intervention, and counseling for individuals, families, and groups. Your role will include bereavement and end-of-life support, as well as timely discharge planning to meet anticipated needs. Requirements

• MSW degree from a CSWE accredited School of Social Work required.
• Current / active Social Work License required or successful pass of the social work licensing exam within 9 months of employment.
• Clinical Social Worker (CSW) OR Licensed Social Worker (LSW)
• Act 31, Act 33, Act 34, Act 73 FBI

Responsibilities

• Provide resource-specific information and facilitate referrals for patients and families.
• Mentor social work students and serve as a field instructor for MSW students.
• Utilize therapeutic counseling methods, including crisis intervention and psychosocial support.
• Educate patients and families on reimbursement systems and advocate for continued care authorizations.
• Assess and intervene with medically and psychosocially complex patients.
• Monitor and reassess the plan of care throughout the hospital stay.
• Collaborate with the healthcare team and community resources to develop individualized care plans.
• Educate and advocate for patients and families on medical care decisions, discharge planning, and end-of-life care.
• Complete comprehensive psychosocial assessments considering various factors and support systems.
