About The Position

Provides face-to-face psychosocial assessments, counseling, and resource coordination for patients and families. Collaborates with healthcare teams to ensure safe, timely, and comprehensive care, including complex discharge planning, crisis intervention, and support for at-risk populations. Acts as a liaison for child and adult protective services, guardianship, foster care, and community resources. Requirements

  • Master’s Degree in Social Work (MSW) required.
  • Permanent LSW licensure required.
  • Certification in Crisis Prevention Intervention or equivalent within 6 months of hire.
  • Knowledge of evidence-based psychosocial assessment, crisis intervention, and short-term counseling.
  • Understanding of disease processes, human behavior, and health-related social needs.
  • Strong discharge planning and decision-making skills.
  • Familiarity with legal mandates related to protective services.
  • Experience working with diverse populations and care transitions.
  • Ability to build collaborative relationships with external agencies.
  • Proficiency in electronic medical records and documentation.
  • Ability to use de-escalation techniques and conduct violence response when needed.
  • May require on-call and holiday work.

Responsibilities

  • Conduct psychosocial assessments and provide therapeutic interventions.
  • Counsel patients and families on emotional, social, and financial impacts of illness/disability.
  • Access and mobilize community and family resources to meet patient needs.
  • Serve as a primary contact for abuse, neglect, and domestic violence cases; coordinate with protective teams.
  • Provide crisis intervention and de-escalation, supporting safety during acute situations.
  • Facilitate complex discharge planning and goals of care conversations.
  • Maintain strong working relationships with hospital staff, law enforcement, community agencies, and legal entities.
  • Document thoroughly in electronic medical records.
  • Respond to and support violence de-escalation, including on-call and holiday rotations.
