About The Position

Park Avenue Center is looking for a Social Worker to work within a supportive and nurturing environment. This position works with clients to obtain appropriate, necessary resources. The Social Worker coordinates and collaborates with clients, clinical staff, community-based resources and others to help obtain stable, supportive housing and additional resources for clients upon transitioning out of treatment. Requirements

  • Must be LSW or MSW, currently licensed in the State of Minnesota.
  • Must be able to maintain positive public relations with referring agencies and the business community at large.
  • Must possess good verbal skills and present a positive professional image.
  • Must have a working knowledge of issues regarding confidentiality.
  • At the time of hiring, must be free of problematic substance use for at least the two years.

Nice To Haves

  • 2,000 supervised hours working with individuals with substance use disorders preferred but not required.
  • Prior experience with an Electronic Health Records system strongly preferred.

Responsibilities

  • Develops and maintains working knowledge of community resources available to clients, including: health insurance options, housing, medication resources and support groups.
  • Participates in client staffing to maintain consistent communication with counseling staff.
  • Meets individually with clients determining needs and facilitating resources; helping clients in their transition from intensive outpatient programming.
  • Facilitates a variety of groups including, but not limited to, activity and psychoeducational.
  • Meets with families, probation, and social workers to develop transition and contingency plans.
  • Maintains appropriate charting to communicate clients’ transitional plans.
  • Coordinates correspondence with Probation, CPS, and other concerned persons.
  • Performs other duties as assigned.
