About The Position

Indiana University Health is seeking an experienced Medical Social Worker to provide psychosocial support, discharge planning, and crisis intervention for patients within the Endocrinology department. This full-time role offers a collaborative environment dedicated to delivering patient-centered care. The Social Worker will: Communicate and collaborate effectively with healthcare team members to facilitate safe, timely, and comprehensive patient care Interpret and address complex patient and family needs, modeling caring practices to caregivers Assess psychosocial risk factors through evaluation of prior functioning, support systems, reactions to illness, and coping skills Counsel patients and families on emotional, social, and financial impacts of illness or disability Access and mobilize family and community resources to meet patient needs Conduct mental health assessments upon physician referral Support complex discharge planning and care transitions Serve as the primary point of contact for cases involving child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence Act as liaison with child and adult protective teams (APS/CPS) Provide intervention in guardianship, foster care, adoption, and mental health Level II placements

Requirements

  • Master's Degree in Social Work (MSW) required
  • Permanent Indiana LSW licensure required, with LCSW preferred
  • Temporary LSW licensure considered with the expectation of obtaining full licensure within the first year of employment
  • 0-3 years of healthcare, mental health, or community-based social work experience required
  • Psychosocial assessments, short-term therapeutic counseling, and crisis intervention techniques
  • Understanding of disease impact and human behavior
  • Knowledge of legal mandates related to protective services (child and adult)
  • Principles of growth, development, and end-of-life care
  • Strong relationships with community agencies, law enforcement, schools, and healthcare teams
  • Proficiency in Microsoft Excel and Word
  • Effective communication and active listening
  • Crisis de-escalation and safety management techniques
  • Strong collaboration within multidisciplinary teams
  • Confidentiality and HIPAA compliance
  • Compassionate, culturally sensitive care delivery

  • De-escalation certification preferred
