Requirements
- Be licensed in the state of Idaho as a Licensed Social Worker (LSW), Licensed Master’s Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW).
- Be BLS CPR certified
- Meet all licensing requirements related to your clinical license
Nice To Haves
- Home Health and Hospice experience preferred, but not required
Responsibilities
- Heritage Home Health Licensed Social Workers (SW) work with patients, their families, and other health care professionals in the patients’ place of residency, performing visits and completing coordination of care.
- The SW follows agency, state, and federal policies and procedures.
- The SW works in an interdisciplinary team and coordinates with other agencies in the community to connect care for patients and their families.
