The Role: Social Worker (LSW) - Family Medicine Institute The Opportunity: MaineGeneral Health is offering an exciting opportunity for an individual to join our Augusta Family Medicine Institute team as a full-time Social Worker. If you have a passion for service excellence and want to work among excellent colleagues…we want to hear from you! This position is 32 hours a week, Monday through Friday, with daytime hours.

Requirements

Maine state licensure as a Licensed Social Worker (LSW) or Licensed Master Social Worker (LMSW) is required.

Responsibilities

Provides case management and coordination for patients and/or families needing a variety of educational, social, and advocacy services.
Researches, develops, and delivers educational materials related to patient and family support based on learning styles and needs of populations served.
Assists patients in developing support systems, linking patients and families to community resources as appropriate.
Works collaboratively with staff, payers, and external resources to identify needs, cultivate relationships with internal/external resources, and enhance clinical outcomes.
