As a member of the hospice team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is full-time Monday- Friday 8a-4:30p.
Requirements
- Master's degree in Social Work is required.
- NYS LMSW licensure is required.
- Minimum of 2-3 years of experience in Medical, Psychiatric, or Health Care setting
Responsibilities
- Diagnose and treat psychological, behavioral and emotional disorders
- Provide specialized skills and mental health therapy for clients
- Identify barriers which impact optimal patient functioning
- Ensure patients receive appropriate resources in most appropriate setting
