About The Position
Part Time Social Worker - Hospice Albany The Community Hospice has a Part-time opportunity for a social worker to join our hospice team caring for patients in homes and nursing facilities as part of our Interdisciplinary Team. As a member of the hospice team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is part-time, scheduled Saturday and Sunday day hours, flexible weekday hours. The Community Hospice provides a thorough orientation and a supportive work environment. Requirements
- NYS SW Licensure.
- Valid NYS Drivers license.
- Minimum 2-3 years experience ideally caring for seniors or those with advance illness.
Responsibilities
- Diagnose and treat psychological, behavioral and emotional disorders as they relate to end of life care.
- Provide specialized skills and mental health therapy for clients.
- Identify barriers which impact optimal patient functioning.
- Ensure patients receive appropriate resources in most appropriate setting.
