About The Position

The Social Work Team Lead acts as a leader within the department, overseeing daily social work activities, providing functional guidance to team members, and ensuring high-quality service delivery. This role involves resolving issues, coaching, training, and educating staff to promote department excellence. The Team Lead collaborates effectively with a multidisciplinary team to facilitate safe, timely, and resourceful patient care, while also acting as a resource for students, interns, and social workers working toward their LCSW.

Requirements

  • Master’s Degree in Social Work (MSW) required
  • Minimum of 5 years in child welfare, mental health and addictions, health, school, or community/organizational settings
  • Permanent Indiana LCSW required
  • Crisis Prevention Intervention (CPI) or an alternative de-escalation training within 6 months of hire required
  • Evidence-based psychosocial assessment, short-term therapeutic counseling, crisis intervention for mental health and substance use disorders
  • Knowledge of disease processes and human behavior related to health-related social needs
  • Complex discharge planning skills to ensure safe patient transitions
  • Legal knowledge of state and county adult and child protective mandates
  • Principles of growth, development, and end-of-life care
  • Ability to identify decision-makers and coordinate goals of care conversations
  • Experience adapting services to geographically and financially diverse populations
  • Ability to develop and maintain strong relationships with hospital leadership, medical teams, law enforcement, APS/DCS, schools, probation, and community agencies
  • Computer literacy and proficiency in documentation within electronic medical records (EMR)

Responsibilities

  • Acts in a lead capacity and serves as liaison to the Social Work Manager or supervisor by overseeing daily social work activities
  • Provides functional guidance, ongoing coaching, and resolves issues within the department to improve outcomes
  • Trains and educates staff on various aspects of the department and social work role to achieve top quality results
  • Participates in service delivery through effective collaboration with the healthcare team
  • Communicates and collaborates with all members of the healthcare team to facilitate safe and timely patient services
  • Interprets and communicates complex patient/family needs, role modeling caring practices to other caregivers
  • Assesses patients' and families' psychosocial risk factors through evaluation of prior functioning levels, support systems, reactions to illness, and ability to cope
  • Counsels patients and families regarding emotional, social, and financial impacts of illness/disability
  • Accesses and mobilizes family and community resources to meet identified needs
  • Provides mental health assessments of patients upon physician referral
  • Provides therapeutic intervention to facilitate complex discharge planning
  • Serves as the point person in child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence cases
  • Acts as liaison between Indiana University Health and state and county child and adult protective teams
  • Provides intervention in guardianship (temporary/permanent), foster care, adoption, and mental health Level II placements
  • May provide specialized mental health services including crisis intervention, addiction assessments, and psychiatric referrals
  • Assists with dangerous or potentially dangerous situations, taking a leadership stance in clinical intervention and de-escalation
  • Supports those experiencing acute crises using de-escalation techniques to help prevent episodes of violence
  • Utilizes effective interviewing principles with victims, perpetrators, families, and significant others
  • Serves as a resource and role model for students, interns, LBSWs, and LSWs
  • Participates with the clinical team in developing treatment plans for psychosocial issues and needs
  • Consults with providers regarding mental health diagnoses and treatment options
  • Develops educational resources for patients and families
  • Acts as a preceptor, sharing knowledge to facilitate learning and professional development
  • Provides clinical supervision for social workers working toward their LCSW
  • Serves as a field instructor for students completing internships and practicums
