About The Position

Assists patients that need a variety of social service interventions. Conducts psycho-social assessments and makes recommendations for post hospital care for patients with chronic conditions who are frequent users of acute care services. Assists patients and their family and/or support systems with coping skills as it relates to patient's chronic illnesses, and helps patients in recovery adjust to the emotional and practical requirements of life. Serves as a mentor to student health coaches. Must be willing to work out of Care4U Fontana and Upland locations as well as Ontario Urgent Care.

Requirements

• A Master’s degree from an accredited school of social work.
• Minimum of two to five years of hospital, medical, and/or field experience required that demonstrates advanced skills and knowledge.
• Knowledge of medical and behavioral disorders symptoms and treatment.
• Ability to evaluate/assess patients with medical and behavioral disorders and develop safe self-care plans and recommendations.
• Ability to comply with mandatory reporting requirements.
• Knowledge of the relationship between medical illness and behavioral health problems related to short and long term interventions and solutions.
• Ability to conduct a variety of social work assessments and interventions.
• Knowledge of community resources.
• Current American Heart CPR card
• Licensed to practice in the state of California (LCSW).
• Computer literate
• Proficient with Microsoft Office Software.
• Must be able to perform the essential physical requirements of the job.
