About The Position

Screenings and assessments of patients working collaboratively with Case Management to facilitate optimal discharge. Grief work and supportive therapy with patient and family regarding end-of-life decisions and critical illness. Function as a resource and referral service to patients and families. Coordination with outside agencies including but not limited to, APC/CPS and psychiatric services. Monday through Friday non-exempt position with weekend call rotation. Assist with supervision of Social Service interns as requested. Participates in community meetings/associations where social service presence is required. Will be certified by Riverside County as designated 5150 evaluator at JFK Memorial Hospital.

Requirements

• M.S.W. and current License as Clinical Social Worker in State of California
• CPR
• Two (2) years experience in an Acute Care Hospital
• Familiarity with psychiatric presentations and mental health services including 5150 criteria and process
• Good communication skills and ability to work with others
• Knowledge of local community resources and procedures for access
