About The Position
Paradise Valley Hospital, a 291-bed acute care hospital in San Diego County, offers comprehensive healthcare and behavioral health services. The Social Worker II position involves providing emotional support, crisis intervention, and counseling to patients across all age groups (Pediatrics, Adolescents, Adults, Older Adults) and their significant others, helping them cope with social and emotional problems related to hospitalization, illness, disability, or trauma. Key duties include information and referral services, facilitating financial assistance, case management, milieu management, conducting psychosocial assessments, contributing to Treatment Plan development, collaborating with a multidisciplinary team, and coordinating discharge planning. The role also requires meticulous documentation and maintenance of clinical records for each client throughout their stay.
Requirements
- Master’s Degree in Social Work field OR Master’s Degree in Marriage and Family Therapy OR Master’s Degree in Professional Clinical Counseling
- In states with a licensure requirement, required upon hire and maintained current in accordance with state regulations
- Minimum one (1) year Inpatient or Outpatient experience with emphasis in mental health care
- Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and childcare, substance abuse, disability, chronic illness, etc.)

Responsibilities
- Provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults, Older Adults, and their significant others to cope with a multiplicity of social and emotional problems
- Provide information and referral
- Provide referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability, or trauma
- Provide case management & milieu management
- Complete psychosocial assessments
- Assist in the development and re-evaluation of Treatment Plans
- Work as a member of a multidisciplinary team
- Coordinate discharge planning
- Complete necessary documentation and maintain clinical record for each client during his/her length of stay
