About The Position

Mountain Pacific is a nonprofit organization established in Montana in 1973, dedicated to partnering within communities to provide solutions for better health. Through federal, state, and commercial funding and partnerships, they advance national public health priorities at local, state, or regional levels. The Social Worker II (MSW) provides advanced psychosocial support and care coordination for individuals living with complex chronic conditions within this mission-driven nonprofit environment. This role supports patients and families in navigating the medical, emotional, and socioeconomic challenges associated with complex chronic conditions. Working collaboratively with interdisciplinary teams and community partners, the Social Worker II assesses complex needs, develops person-centered care plans, and connects individuals to critical resources that improve health outcomes and quality of life. This position also supports the organization’s mission through advocacy, education, and outreach across diverse and underserved populations.

Requirements

MSW (Master of Social Work)
Excellent written and verbal communication
Organizational skills
Attention to detail
Minimum of two (2) or more years of experience in social work, preferably in healthcare, chronic disease management, or nonprofit settings
Ability to travel up to 20%
