The Social Worker II (MSW) provides advanced psychosocial support and care coordination for individuals living with complex chronic conditions within this mission-driven nonprofit environment. This role supports patients and families in navigating the medical, emotional, and socioeconomic challenges associated with complex chronic conditions. Working collaboratively with interdisciplinary teams and community partners, the Social Worker II assesses complex needs, develops person-centered care plans, and connects individuals to critical resources that improve health outcomes and quality of life. This position also supports the organization’s mission through advocacy, education, and outreach across diverse and underserved populations.
