Position Summary: Receives referrals from acute and long term facilities and physician offices for homecare and hospice services. Initiates the referral process. Coordinates arrangements for continuity of care from hospital to home. Performs all duties in a manner consistent with St. Luke’s mission and values.
Requirements
- Master's Degree in Social Work
