About The Position

The Social Worker I is responsible for providing high quality patient care, displaying good skills in assessment, diagnosis, treatment planning, group treatment, individual treatment, family treatment, and discharge planning. Additionally, he/she is expected to function well as a member of the multidisciplinary team and participate in the unit community relations program.

Requirements

  • Masters in Social Work, Psychology, Counseling, Social Science or related field required.
  • 1 -2 years of job-related prior experience in a hospital setting preferred.
  • BLS obtained during orientation period and maintained.
  • De-Escalation and restraint training obtained during orientation period and maintained.
  • Knowledge of counseling theory, crisis interventions, individual, family, and group therapy modalities.
  • Knowledge of the DSM5 and the State of Illinois mental Health and Developmental Disabilities Act.
  • Able to work independently with minimal supervision and exercise independent judgement and initiative.
  • Requires a concentrated level of attention with a High level of organizational skill.
  • Ability to effectively communicate and interact with clients, visitors, and organization personnel and perform in a positive, courteous, flexible, an adaptive manner.

Responsibilities

  • Role Model therapeutic interventions and De-escalation techniques.
  • Completes assigned client's social history within 48 hours of admission and documents in patient chart. Displays good interviewing, diagnostic, and report-writing skills.
  • Conducts individual psychotherapy, family, and group sessions in accordance with state/regulatory standards, displaying good theoretical knowledge of psychopathology and good treatment technique. Provides support, education, and counseling to assist clients and families in resolving issues and adjusting to illness.
  • Completes progress notes and documentation within a timely manner.
  • Completes individualized treatment plans for each patient.
  • Develops discharge plans to meet client's identified needs in conjunction with the client and multidisciplinary treatment team. Engages in timely, efficient discharge planning.
  • Involves collaborative communication with clients PCP/Psychiatrist/Therapist, as needed.
  • Shows good knowledge of referral/placement process and available community resources.
  • Makes timely referrals to community agencies to promote continuity of care for clients.
  • Serves as liaison between the client, family and community agencies.
  • Actively participates in the multidisciplinary treatment planning to discuss and recommend interventions for clients and families, if appropriate.
  • Provides feedback to supervisor on ongoing basis in regards to concerns, improvements, changes, etc.
  • Identifies areas needing improvement and utilizes the facility performance improvement process.
  • Utilizes feedback regarding performance and uses supervision to improve performance.
  • Assists in the development of department goals and process improvement initiatives.
  • Supports and demonstrates the organization mission and philosophy through self-examples.
  • Encourage therapeutic milieu.
  • Maintain current State of Illinois licenses and credentials.
  • Performs all other duties as assigned.
