Job Overview Responsible for AltaMed Managed Care participants assigned to the team. This position works closely with providers and nurses to coordinate services and advocate for participants.
Requirements
- Bachelors’ or master’s degree in social work from an accredited university required.
- Minimum of 2 years of experience in case management and social advocacy required.
- Licensed Clinical Social Worker also accepted.
- Bilingual English/Spanish strongly preferred.
Nice To Haves
- Homeless population experience strongly preferred.
- Managed Care Experience preferred.
