About The Position

Catholic Charities is seeking a self-motivated leader who is compassionate, trauma-informed, and culturally aware to serve as a Team Lead on its Medica team. They will work respectfully with children, adolescents, persons in poverty, and people of diverse racial and cultural backgrounds. They are responsible for case management services, short-term counseling, and assisting clients with access to supportive services, and helping clients set goals for themselves and their families. They will provide ongoing aftercare to clients and their families. They will provide comprehensive counseling to clients of Catholic Charities per program needs and must be able to work effectively with clients with complex psychosocial concerns including mental health issues, unstable housing, unemployment, lack of medical insurance, dysfunctional relationships, and legal concerns. They may conduct support/education groups.

Requirements

  • Must be in good standing and meeting all performance expectations of the Social Worker I role.
  • Current MN Social Work License (LSW/LGSW/LISW/LICSW)
  • Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
  • CPR/First Aid Certification required.

Nice To Haves

  • One (1) year of broad-based clinical experience preferred.
  • One (1) year of management experience, including supervisory experience preferred
  • Experience in individual, family, and group counseling preferred.
  • One year of experience as a case manager/care coordinator, preferably in health plan care coordination or county waiver case management

Responsibilities

  • Provide intake and comprehensive case management services including: Intake, interview and assessment Goal setting Information and referral services Individualized client case management plan
  • Provide assessment services and short-term counseling, skill building techniques, and aftercare to individuals, couples and families.
  • Provide client services in a variety of outreach sites in the community and division facilities as assigned. Conduct support/education groups as needed.
  • Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes.
  • May provide behavioral management support and consultation to collateral sources and coordinate joint efforts to address needs or problems of the client population being served.
  • Make appropriate referrals and provide comprehensive decision-making counseling to help ensure client’s needs are being met.
  • Develop cooperative working relationships with communities including schools, parishes, and other professionals on behalf of the clients we serve. Work collaboratively with outside agencies to provide behavior management support and consultation.
  • Carry a caseload of clients per direction of supervisor
  • Complete one or two chart reviews per staff person per month at the direction of the supervisor
  • Establish regularly scheduled times in the work week where staff can call, email, instant message or meet in person to discuss client cases or ask questions
  • Assist supervisor with the training of new Social Workers on their team, including shadowing and being shadowed on home visits with the new Social Worker, training new Social Workers on how to complete paperwork, and answering questions as they arise from new staff
  • Facilitate or assist with the facilitation of team meetings at supervisor’s request
  • Other duties as assigned
