Baylor Scott and White is the largest not-for-profit healthcare system in Texas that empowers you to live well. The Hospice Social Worker oversees social work care for patients and families. They provide education and advice to cope with emotional or physical issues. Coordination includes crisis intervention, bereavement, client advocacy, referrals, education, discharge planning, counseling, and psychosocial assessments.

Requirements

  • Demonstrates critical communication skills including self-awareness, inquiry/dialogue, conflict management, negotiation, advocacy and listening.

  • Know the standards and methods to restore or enhance social, psychosocial, or bio-psychosocial functioning of people, couples, families, groups, organizations, and communities.
  • Recognize that resources may be limited. Consider factors related to safety, effectiveness, and efficiency in planning and delivering patient care.
  • Able to develop therapeutic relationships with patients and family and maintains appropriate boundaries.
  • Function autonomously in the community. Able to drive own vehicle and travel within the entire office service area as needed.
  • Functions as a team member and works collaboratively and autonomously, involving intra and inter-disciplinary work with colleague and community.

  • 1 Year of Experience
  • Lic Clinical Social Worker (LCSW), Lic Masters Social Worker (LMSW): Must have one of the following licenses: Lic Masters Social Worker (LMSW) or Lic Clinical Social Worker (LCSW).

Responsibilities

  • Provides patients and family members with complex education and advice. This helps them cope with emotional or physical problems. This is outlined by the Texas State Board of Social Worker Examiners scope of practice.
  • Accountable for developing a written care plan for each patient, family, and caregiver before providing services. Care coordination among team members is shown through team conference notes, clinical notes, and phone communication in the clinical record.
  • Helps a patient and family with finding alternate means for help for basic life needs.
  • Conducts patient assessments as required by Medicare Conditions of Participation and documents in the medical record.
  • Coordinates patient respite stays at local facilities to include contracting, transportation and clinical staff for safe patient transfers to/from facility.
  • Serves as a liaison with local community resources and staff. Ensures a smooth transition and placement of hospice patients.
  • Helps with admissions, discharges, transferring, arranging alternate levels of care, and attending the deaths of the agency patients as needed.
  • Participates in planning and helps with the Hospice Annual Memorial service for families of patients who passed away in the past 12 months.
  • Coordinates with Texas Adult Protective Services to report patient safety issues or assist in investigations about patient abuse and safety.
  • Performs other position appropriate duties as required in a competent, professional and courteous way.
