About The Position

The Social Worker is assigned to the Geriatrics & Extended Care Service in the Home Based Primary Care Program and functions as a member of a multidisciplinary team. The Social Worker will provide comprehensive, longitudinal, interdisciplinary, primary care in the homes of Veterans with serious medical, psychosocial, and behavioral conditions for whom routine clinic-based care is not effective or may not be possible.

Requirements

  • Must maintain medical clearance from the employee health department
  • Have a valid state driver's license
  • Provide proof of insurability
  • Successfully complete all facility requirements to drive a GSA Vehicle and transport Veterans.
  • Successfully complete all facility required TMS trainings to drive a GSA Vehicle and transport Veterans.

Responsibilities

  • Develops an assessment of the Veteran under close supervision of an independent or advanced practice licensed Social Worker and in collaboration with the interdisciplinary treatment team, the Veteran, and, whenever possible, family members and significant others.
  • Develops the social work treatment plan and sets achievable and measurable treatment goals with the Veteran and/or family in collaboration with the HBPC interdisciplinary treatment team members and under the close guidance of the social work clinical supervisor.
  • Provides treatment planning for mental illness and psychosocial challenges which includes diagnosis, individual psychotherapy, and supportive counseling to be provided in the Veteran's home under the close supervision of an independent or advanced practice licensed Social Worker.
  • Uses knowledge and theories of mental health and aging to independently perform comprehensive initial and follow-up assessments to meet the psychosocial needs of the patient and their support systems.
  • Develops the psychosocial plan of care, following advanced assessment and screening, to improve the patient's ability to achieve optimal psychosocial functioning.
  • Independently responsible for crisis intervention and/or suicide prevention in home environments.
  • Performs rapid assessments and develops crisis management plans.
  • Assesses for suicidality and self-harm, perform appropriate interventions, and create a safety plan for the Veteran.
