About The Position

Social Work Services at the Mount Sinai Hospital The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities. The Social Worker, under general supervision, assists patients and their families in understanding illness and disabilities and resolving personal, financial and environmental difficulties which interfere with obtaining maximum benefits from medical care. Specific duties may vary by area assigned. The Social Worker develops collaborative relationships with other disciplines to assure effective patient care. After appropriate training, demonstrates competency for specific patient population. In specific program areas (e.g. Renal and Transplant), more detailed job descriptions specific to the program area have been developed.

Requirements

  • Master's degree in social work from an accredited institution.
  • Knowledge of social work services theories and techniques.
  • Level of knowledge of social work process and community resources and agencies in relation to health, illness and disability, generally acquired through six months to one year related experience in a medical or health care setting.
  • LMSW or LCSW

Responsibilities

  • Engage patients/care partners to assess and evaluate psychosocial needs for discharge planning and/or care management and/or other services.
  • Engage patients/care partners for adjustment to health issues and evaluate coping strategies.
  • Formulate interventions, discharge and/or comprehensive care plan that address identified needs.
  • Provide clinical support to achieve mutually identified goals, in accordance with social work practice standards.
  • Connect patients/care partners with appropriate community resources.
  • Assess patterns of need within population(s) and work collaboratively to address them.
  • Engage providers/interdisciplinary team around patient/care partner treatment concerns and provision of services.
  • Collaborate with other internal departments and external community-based organizations and providers, as needed.
  • Document patient encounters and program activities accurately, with appropriate detail, in a timely manner.
  • All documentation follows department and program specific requirements, provides a concise narrative, and is current, meaningful and internally consistent.
  • Participate actively in supervision to assess and improve professional/clinical competence and actively seek professional development opportunities.
  • Pursue professional development delineated by department Clinical Career Ladder.
  • Obtain licensure consistent with New York State Office of the Professions criteria and departmental expectations.
  • Performs other related duties
